Vitamin B12 Protects Against Dementia- Even in the Absence of a B12 Deficiency

A new research study published in the Journal Neurology found that people aged between 61 and 87, with higher blood levels of vitamin B12, were six times less likely to experience brain shrinkage (which is associated with dementia) then people with lower levels of B12 in their blood.

Prior studies on the link between B12 and dementia focused on how increased levels of B12 and other B vitamins were linked to low levels of homocysteine, an amino acid that increases risk of dementia and cognitive impairment.

This study, which followed 107 volunteers, found another connection between B12 and the brain- the prevention of brain shrinkage.

The best sources of B12 are meat, fish, milk and eggs. Unfortunately these foods are problematic for people with health challenges as they are foods of animal origin that tend to be more difficult to digest.  Furthermore, such foods are often tainted with pesticides (stored in animal fat), heavy metals, and hormones.

Interestingly enough, people studied in this trial were NOT shown to be deficient in vitamin B12 using standard blood tests.  In my practice, I will test for B12 just to make sure a patient does not show elevated levels, before prescribing subcutaneous vitamin B12 shots, or vitamin B12 supplementation (often in the form of a complete multivitamin formula).  However, a patient certainly does not need to show a B12 deficiency to derive benefit from this vitamin. B12 therapy is effective for weight loss, mental function, energy and depression.

Study details

Vogiatzoglou and co-workers recruited the community-dwelling volunteers and collected blood samples to measure levels of vitamin B12, holotranscobalamin (holoTC), methylmalonic acid, total homocysteine, and folate. Brain volume loss per year was calculated using MRI scans.

Over five years of study, the researchers noted a greater decrease in brain volume among people with lower vitamin B12 and holoTC levels.

Indeed, people with lower levels of B12, defined as blood levels below 308 picomoles per litre, were six times more likely to experience brain shrinkage, said the researchers. On the other hand, high levels of methylmalonic acid or homocysteine, or low folate levels were not linked to brain volume loss.

“Low vitamin B12 status should be further investigated as a modifiable cause of brain atrophy and of likely subsequent cognitive impairment in the elderly,” stated the researchers.

It is important to note that none of the volunteers were (known to be) vitamin B12 deficient.

“Previous research on the vitamin has had mixed results and few studies have been done specifically with brain scans in elderly populations. We tested for vitamin B12 levels in a unique, more accurate way by looking at two certain markers for it in the blood,” said Vogiatzoglou.

The potential of supplements

Commenting independently on the results, Dr Susanne Sorensen from the Alzheimer’s Society said: “Shrinkage is usually associated with the development of dementia.

“As vitamin B may be given as a food supplement, it may be useful to include tests of vitamin B levels in the general assessment of health of older individuals.

“This is another example of why it is crucial for people to lead a healthy lifestyle with a balanced diet rich in B vitamins and antioxidants.”

B12 for brain

A University of Oxford study reported in 2007 that increased vitamin B12 levels may reduce the rate of age-related cognitive decline and dementia by 30%.

The study, published in the American Journal of Clinical Nutrition, followed 1648 participants for 10 years and also found that increased levels of the amino acid homocysteine doubled the risk of dementia or cognitive impairment.

Previously, epidemiological studies have reported that high levels of the amino acid homocysteine are associated with suspected or confirmed dementia. Indeed, the Framinghamstudy reported that people with homocysteine levels above 14 micromoles per litre of serum had twice the risk of dementia.

As a Naturopathic Medical Doctor who prescribes bioidentical hormones to patients who need them I feel very strongly about protecting the rights of my patients and their ability to obtain the safest most efficacious means of treating hormone imbalances.

The Value of Estriol for Women

Many women rely on the benefits of compounded, bio-identical hormones that include estriol, the safest and most protective form of estrogen that is naturally made by the body.  In January of this year, the FDA ordered pharmacies to ban the use of estriol even though 80% of bio-identical hormone replacement therapy prescriptions use it, and estriol is a hormone that is naturally manufactured by the human body.  The FDA feels that estriol is a “new and unapproved drug” and hence its safety is unknown.  However, according to the HOME coalition, the FDA has admitted that no adverse event involving compounded bio-identical estriol has ever been reported.  A research study supported by the Department of Defense followed 15,000 women for 40 years, monitoring their estriol levels. This landmark study found that the women with the highest levels of estriol had a 58% reduced risk of developing breast cancer.  And women whose estriol levels rose the highest during pregnancy (up to 1,000 fold) had the lowest risk of breast cancer.  It is also interesting to note that, as a group, Asian women have some of the highest levels of estriol in their body, and the least incidence of breast cancer.  The lead researcher for this study, Dr. Pentii K. Siiteri, PhD,  remarked that the finding from this study “suggests that estriol might be an effective option for either breast cancer prevention or treatment regimens.”

Meditation and Nutraceuticals- a True Prescription for High Blood Pressure

Today’s NPR morning edition,  “To Lower Blood Pressure, Open Up and Say ‘Om'”,  discussed research demonstrating that a daily meditation practiceincreases nitric oxide levels, which opens up blood vessels, and decreases blood pressure.  A physician interviewed on the show, Dr. Zusman, talked about how he was able to reduce his patients blood pressure prescription medications.  Taking it a step further, it would be helpful to interview licensed Naturopathic Physicianswho recommend meditation together with nutrients and herbs proven to maintain healthy blood pressure levels. The Gluten-Free Diet Fad?

There is a lot more talk out there these days about the dangers of gluten.  Some people have full blown celiac disease, where their body can be severely damaged by ingesting gluten. Others have a mild, moderate or severe intolerance to gluten and gliadin in their diet.  At LTP Natural Medical Center I test patients for gluten intolerance through an IgG blood test that looks for specific foods that the body mounts an immune response to.  When those foods are eliminated from the diet, patients feel better. And gluten is often on the list.  So while some skeptics will say that gluten-free is just a fad, I do not agree.  This is a very real condition, even for patients not diagnosed with celiac disease.  Going deeper, however, I find that if we focus on testing for, and repairing the integrity of the digestive tract, these food intolerance’s fall away.  So if we want to go to the root cause, I would say compromised digestion is the place to look.

Taking a Summer Break from ADHD Meds

Summer is a perfect time for kids to take a break from their ADHD meds. Some experts feel that taking a medication vacation when your child is heading off to summer camp can be problematic. This is because with psychotropic medications like ADHD drugs and antidepressants, symptoms can get worse when you stop the medication-even worse then they were when you first started the treatment.

    True Prevention Means More Than Early Disease Detection

    An ounce of prevention when it comes to health is worth its weight in gold. I admire the handful of patients that come through our medical practice that don’t have a disease or symptoms they want to get rid of.  They are interested in developing a healthcare partnership with their Naturopathic Medical Doctor and a lifestyle plan to optimize their health and well-being.

    And the Centers for Disease Control and Prevention (CDC) are realizing the power of this approach to healthcare on a national scale.  They have launched the “Healthiest Nation Campaign” in hopes of elevating our country’s health ranking.  According to the CDC, we don’t even rank in the top 10…and on one list it’s 26th; on another, 47th.

    Julie Gerberding, Director of the Centers for Disease Control and Prevention says  the US puts “way too much emphasis on treating disease rather than protecting health in the first place.”

    According to Gerberding, when we talk about prevention in this country, we are usually talking about early detection of disease, and that’s secondary prevention.  She feels that “we’re not really talking about the things we need to do before we get to the doctor’s office.”

    To do that, we must get back to the basic foundation of a healthier lifestyle. Simple steps include:

    • Healthier diet
    • More movement
    • Less TV
    • Less video games
    • More quiet time in nature

    Doctor moves beyond ‘fixing the broken piece’

    This article discusses the transition that one primary care doctor made from a typical medical practice, seeing 25-30 patients per day to an integrative medical practice where she focuses on treating the underlying cause of symptoms rather then applying a band-aid to the problem.  She took on line courses and three one-week sessions to learn about integrative medicine.

    Unfortunately many people are not aware that Licensed Naturopathic Medical Doctors practice primary care medicine and integrative medicine as the base of their practice. To properly practice this type of medicine Licensed Naturopathic Medical Doctors spend four years in medical school, beyond their undergraduate work, and take medical boards in order to receive their license and practice integrative medicine. Big difference. This specialized group of physicians is actually trained on how to practice integrative, holistic, natural medicine for four years, on campus (not on line) interacting with the leaders in the field of integrative medicine throughout their medical education.

    I seem to end up with a lot of patients in our practice who went to a traditional MD, like the doctor in this article, that then “converted” to a more holistic approach to medicine.  They are usually patients with complex chronic illnesses, that did not see the results they were looking for when working with an MD who learned about natural medicine through a satellite program or continuing education classes.  In these cases, fixing their diet and lifestyle and testing for heavy metals while using acupuncture didn’t cut it.

    Doctors like Kinigakis, the physician highlighted in the article, serve an important role in our evolving health care system and it is great that they are tipping the scale back to a more balanced view of the body and how it can heal itself.

    Experience with patients, and with colleagues like Kinigakis, that I teach around the country, reveals that licensed  Naturopathic Medical Doctors (NMDs or NDs) who received their education at a four year CNME approved Naturopathic Medical School, have a different level of training and a more in-depth understanding and appreciation for how to use the tools that natural medicine has to offer.  It seems that NMDs are operating in a different paradigm entirely, rather then trying to fit natural medicine into the existing and prevailing medical paradigm of our times.  And that leads to a different level of success when working with  patients who are ready to heal.

    Cholesterol Lowering Drugs for Kids?

    The American Academy of Pediatrics issued a new recommendation on Monday that statin medications should be used in some children to help prevent heart attacks later in life.  And many pediatricians are furious at the Academy for making this recommendation.

    High cholesterol, even when it runs in the family, can be lowered to a level that reduces heart attack risk..without taking a statin drug.  There is also no proof that statin medications prevent heart attacks in children.  And even in adults, we know that people who take statin drugs get heart attacks and people who do not take statin drugs get heart attacks. Furthermore, statin medications deplete the body of heart protective nutrients such as CoQ10. They also block the formation of essential hormones that are important for lifelong health.

    As a doctor who treats children, it is concerning that the Food and Drug Administration approved the use of a statin medication, Pravachol, in children without evidence of the safety of using the medication at a young age, before the hormone system has fully matured.

    Simple changes in diet and lifestyle are a wise first step towards preventing heart attacks later in life, especially for children.  Here are some basic steps to take:

    • Remove ALL trans fatty acids from the diet. This involves reading labels and looking for the words “partially hydrogenated oil,” “hydrogenated oils,” or “trans fatty acids.” Even if the label says “zero trans fats” there can still be trace amounts found in the product. So read the ingredient list before offering the food to a child. Hydrogenated fats increase your risk for heart disease and heart attacks and also damages blood vessels.
    • Limit sugar intake
    • Include one serving of raw vegetables or fruits in every meal. When you look at a child’s plate, there should be something raw on that plate- whether it is apple slices, salad, carrots, or any other raw fruit or vegetable that the child will eat.
    • Encourage at least 1 hour of physical activity per day
    • Teach children how to handle stress in a healthy way. Set aside one hour of stress-relieving activity per day-through quiet time away from computers, TV and video games. Activities such as yoga, playing with an animal, or spending time in nature are all proven methods to relieve stress.
    • Have your child evaluated for nutritional deficiencies by a licensed Naturopathic Medical Doctor.  Nutrient deficiencies like Vitamin D3 and Omega 3 Fatty Acids can increase a child’s risk of heart disease.
